Sedikit info mengenai sakit kepala (migraine)


Definition

A migraine headache can cause intense throbbing or pulsing in one area of the head and is commonly accompanied by nausea, vomiting, and extreme sensitivity to light and sound. Migraine attacks can cause significant pain for hours to days and be so severe that all you can think about is finding a dark, quiet place to lie down.
Some migraines are preceded or accompanied by sensory warning symptoms (aura), such as flashes of light, blind spots or tingling in your arm or leg.
Medications can help reduce the frequency and severity of migraines. If treatment hasn't worked for you in the past, talk to your doctor about trying a different migraine headache medication. The right medicines, combined with self-help remedies and lifestyle changes, may make a tremendous difference.

Hari ni nak share sikit pasal  Carpal Tunnel Syndrome...penyakit ni selalunya terjadi pada mereka yg bekerja banyak menggunakan tangan terutama menaip..so kalau nak menaip kena dengan cara yang betul..elakkan bg tekanan pada tempat2 tertentu yang boleh menyumbang kepada masalah ini.


Definition

Carpal tunnel syndrome is a progressively painful hand and arm condition caused by a pinched nerve in your wrist. A number of factors can contribute to carpal tunnel syndrome, including the anatomy of your wrist, certain underlying health problems and possibly patterns of hand use.
Bound by bones and ligaments, the carpal tunnel is a narrow passageway — about as big around as your thumb — located on the palm side of your wrist. This tunnel protects a main nerve to your hand and nine tendons that bend your fingers. Compression of the nerve produces the numbness, pain and, eventually, hand weakness that characterize carpal tunnel syndrome.
Fortunately, for most people who develop carpal tunnel syndrome, proper treatment usually can relieve the pain and numbness and restore normal use of their wrists and hands.

Symptoms

Carpal tunnel syndrome typically starts gradually with a vague aching in your wrist that can extend to your hand or forearm. Common carpal tunnel syndrome symptoms include:
  • Tingling or numbness in your fingers or hand, especially your thumb and index, middle or ring fingers, but not your little finger. This sensation often occurs while holding a steering wheel, phone or newspaper or upon awakening. Many people "shake out" their hands to try to relieve their symptoms. As the disorder progresses, the numb feeling may become constant.
  • Pain radiating or extending from your wrist up your arm to your shoulder or down into your palm or fingers, especially after forceful or repetitive use. This usually occurs on the palm side of your forearm.
  • A sense of weakness in your hands and a tendency to drop objects.
When to see a doctorIf you have persistent signs and symptoms suggestive of carpal tunnel syndrome, especially if they interfere with your normal activities and sleep patterns, see your doctor. If you leave the condition untreated, nerve and muscle damage can occur.

Causes

Carpal tunnel syndrome occurs as a result of compression of the median nerve. The median nerve runs from your forearm through a passageway in your wrist (carpal tunnel) to your hand. It provides sensation to the palm side of your thumb and fingers, with the exception of your little finger. It also provides nerve signals to move the muscles around the base of your thumb (motor function).
In general, anything that crowds, irritates or compresses the median nerve in the carpal tunnel space can lead to carpal tunnel syndrome. For example, a wrist fracture can narrow the carpal tunnel and irritate the nerve, as can the swelling and inflammation resulting from rheumatoid arthritis.
In many cases, no single cause can be identified. It may be that a combination of risk factors contributes to the development of the condition
Risk factors

A number of factors have been associated with carpal tunnel syndrome. Although by themselves they don't cause carpal tunnel syndrome, they may increase your chances of developing or aggravating median nerve damage. These include:
  • Anatomic factors. A wrist fracture or dislocation that alters the space within the carpal tunnel can create extraneous pressure on the median nerve. Also, carpal tunnel syndrome is generally more common in women. This may be because the carpal tunnel area is relatively smaller than in men and there's less room for error. Women who have carpal tunnel syndrome may also have smaller carpal tunnels than women who don't have the condition.
  • Nerve-damaging conditions. Some chronic illnesses, such as diabetes and alcoholism, increase your risk of nerve damage, including damage to your median nerve.
  • Inflammatory conditions. Illnesses that are characterized by inflammation, such as rheumatoid arthritis or an infection, can affect the tendons in your wrist, exerting pressure on your median nerve.
  • Alterations in the balance of body fluids. Certain conditions — such as pregnancy, menopause, obesity, thyroid disorders and kidney failure, among others — can affect the level of fluids in your body. Fluid retention — common during pregnancy, for example — may increase the pressure within your carpal tunnel, irritating the median nerve. Carpal tunnel syndrome associated with pregnancy generally resolves on its own after the pregnancy is over.
  • Workplace factors. It's possible that working with vibrating tools or on an assembly line that requires prolonged or repetitive flexing of the wrist may create harmful pressure on the median nerve, or worsen existing nerve damage. But the scientific evidence is conflicting and these factors haven't been established as direct causes of carpal tunnel syndrome. There is little evidence to support extensive computer use as a risk factor for carpal tunnel syndrome, although it may cause a different form of hand pain.

  • Tests and diagnosis
  • Your doctor may conduct one or more of the following tests to determine whether you have carpal tunnel syndrome:
    • History of symptoms. The pattern of your signs and symptoms may offer clues to their cause. For example, since the median nerve doesn't provide sensation to your little finger, symptoms in that finger may indicate a problem other than carpal tunnel syndrome. Another clue is the timing of the symptoms. Typical times when you might experience symptoms due to carpal tunnel syndrome include while holding a phone or a newspaper, gripping a steering wheel, or waking up during the night.
    • Physical exam. Your doctor will want to test the feeling in your fingers and the strength of the muscles in your hand, because these can be affected by carpal tunnel syndrome. Pressure on the median nerve at the wrist, produced by bending the wrist, tapping on the nerve or simply pressing on the nerve, can bring on the symptoms in many people.
    • X-ray. Some doctors may recommend an X-ray of the affected wrist to exclude other causes of wrist pain, such as arthritis or a fracture.
    • Electromyogram. Electromyography measures the tiny electrical discharges produced in muscles. A thin-needle electrode is inserted into the muscles your doctor wants to study. An instrument records the electrical activity in your muscle at rest and as you contract the muscle. This test can help determine if muscle damage has occurred.
    • Nerve conduction study. In a variation of electromyography, two electrodes are taped to your skin. A small shock is passed through the median nerve to see if electrical impulses are slowed in the carpal tunnel.
    The electromyogram and nerve conduction study tests are also useful in checking for other conditions that might mimic carpal tunnel syndrome, such as a pinched nerve in your neck.
    Your doctor may recommend that you see a rheumatologist, neurologist, hand surgeon or neurosurgeon if your signs or symptoms indicate other medical disorders or a need for specialized treatment.


Sakit Sangat

Assalamualaikum....

Semalam seharian terbaring jer...badan terasa amat lesu, letih dan tak bermaya...sakit sangat rasanya bahu dan tengkok ni..sakitnya tidak seperti selalu..berganda2 sakitnya....tambahan en hubby tak ada pulak kat rumah...ikutkan hati nak aje g hospital...

Malamnya, selepas anak2 mkn...makan megi jer...tak larat nak memasak...aku makan ubat yang sebeban doktor bagi dan terus baring...Ya Allah, nak tunggu ubat tu memberikan kesan...sakit yg amat...nak menangis tp aku tahan tak nak tunjuk depan anak2...sms en hubby, but no reply...dia bz la tu pasang pelamin...cuba utk tido tp tak boleh...rasa berdenyut2 yg amat sangat..

Akhirnya, berjaya lelap selepas jam 11mlm...tp tak lena.... 1:45am hp berbunyi...tanda sms masuk..sape lg en hubby la...mengadu kat dia...masa ni rasa sakit still ada..ubat tu mcm tak memberikan kesan pn...

Pagi ni rasa sakit tu masih ada but ada kurangnya...dapat call dr Akedemi Memandu Nilai, terpaksa la batalkan sesi belajar memandu hari ini...sb tidak sihat..burn 2jam mamc tu je...kena la bayar yg lg 2jam tu..

Ya Allah, kau sembuhkanlah kesakitan ini...sesungguhnya banyak tggjwb yg perlu aku laksanakan...blm masanya utk aku berundur Ya Allah...

Part 3 : Admitted Again...Room 422 14/03/2013

ok..kita masuk episod 3 pulak...

18/03/2013, 9.00pagi...tgh dok lepak tgk tv nurse dtg bgtau nak bw g klinik Dr Fauziah...Sampai kat klinik pesakit dah ramai menunggu...tp keutaman akan diberikan kepada pesakit yg dr wad la..so aku jd org pertama masuk jumpa doktor...berdebar2 sb dah lama tak jumpa doktor ni...last jumpa post natal anak bongsu..kira2 3tahun lepas..

Aku    : Assalamualaikum doktor
Doktor : W'salam...hurrmm ju apa citer
Aku     : Sakit tang perut doktor...dulu kan sebelah kiri...skrg sebelah kanan pulak
Doktor : ye la..dulu sebelah kiri tu kita dah release kan adhesion tu..dah ok kan
Aku    : Ha'ah
Doktor  : Berapa byk awak dah buat operation ni ...
Aku   : huih...byk dokto
Doktor  : tahun lepas?
Aku   : 3 kali

past doktor ada la tanya lg beberapa soklan yg berkaitan sakit puan...

Doktor : Meh saya periksa...u dah lama tak buat pap smear kan...kita buat sekali..

Memula doktor buat ultrasound...everythings look ok..ovari ok...rahim ok tak ade bengkak..cuma ada very small cyst jer...lepas tu doktor buat pap smear dan pemeriksaan pelvic...secara keseluruhan semuanya ok...so doktor pn kata ini bkn sb endometriosis ni..tp kita cuba la buat rawatan utk endometriosis...kita bg suntikan...mcm mn...nak tak...saya redha jer doktor...

Settle, balik waad tggu doktor Jasmi pulak..msa kat wad masuk la beberapa sms dr ofis..tanya bila masuk kerja...adoiii...apa la nasib dpt bos cmni..

Ptg baru doktor masuk...so mcm mna...sy gk Dr Fauziah pun kata kemungkinan bkn sb endometriosis tap adhesion colic..sy pn rasa mcm tu...sy pn surrender la...

Aku  : Dah tu, cm ner doktor...sy stil sakit..kalau sakit sikit2 sy boleh jer tahan...sb sakit sangat la saya dtg spital...kalau adhesion colic, so cmne rawatan dia

Doktor  : Adhesion colic ni selalu nya kita biarkan jer...worse come to worse buat surgery la..tp sy tak galakkn sb benda tu akan jadi balik dan sy takut nnti jd lg teruk...

Aku   : hmmm...kalau cmtu tak pe la...nk buat cmner...cuma kalau lepas ni sakit jugak lagi dan makin sakit mcm mana...

Doktor   :Yang tu nnti kita bincang kemudian la...u dtg jer klinik..so u nak balik ke arini

Aku  : semestinya nak balik doktor...

Doktor  : sure ke ni...sakit lg ni mcm mana...nk ambik injection mcm mana...

Aku   : tak pe la doktor, sy nak balik jugak..sebenarnya injection tu tak jalan pn ngan saya..

Doktor  : ok, boleh balik...so u nak mc tak.

Aku  : terdiam seketika...bagi sy mc sehari jer la doktor.

Doktor : dgn muka tak puas atinya...u biar betulll..sehari jer...boleh ke kerja sakit mcm ni...

Aku   : boleh kot....erkkk...esok 19hb kan doktor...

Doktor  : ermmm...ha'ah

Aku  : konfem , mc sehari jer doktor....masuk month end ni , byk report nak kena buat..

Doktor   : ok la kalau cmtu...sy bekalkan u pain killer...

So , hari isnin 18/03/2013 discharge..walaupun dlm keadaan masih sakit...sampai hari ni masih sakit lagi...dan makin hilang selera makan dan minum...mampu minum air kosong jer....namun hati terubat bila anak2 ada disisi...


Part 2 : Admitted Again...Room 422 14/03/2013

Kita sambung kembali cerita yg belum habis dari part 1...last sekali cerita waktu buat colonoscopy...pada 16/03/2013...ptg nya doktor tak masuk (ward review)...so tggu la esoknya...dari pada lepas buat procedure tu sampai ke malam..menu yg spital bg kat aku cuma air + sup kosong , itu sahaja...no nasi atau mknn lain...dah la perut sememangnya dah kosong selepas buat detox malamnya...siang bg makan tu jer...mmg tak lalu la nak makan...huhuhuhuhu...hadap jer la kan.

17/03/2013, 9.45pagi...ward review by doctor...seperti biasa doktor tu dengan muka yang tak lekang dengan senyuman walaupun dah fenin ngan kes aku ni masuk dengan ceria bgtau outcome colonoscopy...

Doktor : Assalamualaikum...so mcm mana hari ni
Aku     : sakit doktor
Doktor : hmmm...saya pun fenin la....colonoscopy smlm semua nya normal..tak ada apa2 masalah
Aku     : ermmmm....(dlm hati berkata, nape saya boleh rasa masa buat procedure tu..sakit plak tu)
Doktor : U rasa mcm mana...u cuba bagi cadangan...
Aku     : hah..saya bg cadangn...mana sy tau doktor...doktor y lebih arif..
Doktor : U ada endometriosis dulu kan...mungkin sb tu kot u sakit...nak tak try jumpa pakar O&G
Aku     : Errkkk....ntahla doktor...saya pn tak tau la...
Doktor : U rasa2...apa pendapat u
Aku     : Doktor la decide sy ikut jer
Doktor : ok...kita dapatkan sec opinion dr O&G...dulu u ngan doktor mana ya
Aku     : Dr Fauziah
Doktor : Sya refer u pada dia...so u nak balik dan esok datang balik jumpa dia ke atau u nak stay n esok pagi terus jumpa dia...yang ni u kena decide la
Aku     : hmmm...sy tak nak 2kali kerja...balik dan datang balik esok...so better saya stay for 1 more night n esok terus jumpa dia...
Doktor  : ok la kalau mcm tu....u rehat la...u nak ubat tahan sakit ke...
Aku      : Nak...nak... doktor..mmg berniat nak request pun td...makin sakit lepas buat colonoscopy smlm...
Doktor  : ok la..kita bg injection "Tramal"
Aku   : huhuhuhu...redha (dlm hati)

Tghari tu, nurse pun menjalankan tugasnya memberi suntikan kat aku...hadoiiiyaiii....kiri kanan kena inject ni..bermalam lagi di hospital...hati dah mula merindui anak2...
